 IS DENTAL AMALGAM A GOOD ALTERNATIVE FOR A  DENTAL FILLING?

Although amalgam fillings may be place in variety of applications in dental offices in the united States, even today, they are generally of limited value in today’s cosmetically conscious dental patient. The dental amalgam is historical referred to as the “gray filling” and unfortunately tend to stain and discolor teeth with time due to their corrosion.   We do not feel the dental amalgam offers any significant benefit for the patient in terms of longevity, strength, or even cost.   The cosmetic value of the dental amalgam poor and not a viable option for cosmetic treatment.

Health considerations of  dental amalgam are being debated among researchers and health organizations, and are being considered much more carefully, due to the nearly 50% mercury levels in this type of restoration.  Although recent findings by the FDA were issued as of July 28, 2oo9, its final regulation on Dental Amalgam.

“The U.S. Food and Drug Administration today issued a final regulation classifying dental amalgam and its component parts – elemental mercury and a powder alloy—used in dental fillings. While elemental mercury has been associated with adverse health effects at high exposures, the levels released by dental amalgam fillings are not high enough to cause harm in patients.

The regulation classifies dental amalgam into Class II (moderate risk). By classifying a device into Class II, the FDA can impose special controls (in addition to general controls such as good manufacturing practices that apply to all medical devices regardless of risk) to provide reasonable assurance of the safety and effectiveness of the device.“

It is the Possition of the FDA, that health associated with mercury levels release by dental amalgam  is not high enough to cause harm to patients.  However, the FDA raised the classification of Dental Amalgam to a class 2 risk  ( II moderate risk).

 Its my  personal feeling as a father and a dentist,  that dental amalgam should not generally be continued to be utilized in routine dental applications.   The mercury / silver fillings’ high level mercury and its potential future health considerations makes this choice of restoration  a questionable alternative in almost any situation.

Tooth Colored Cosmetic Composite Fillings & There Applications

The dental composite filling, is a white tooth colored filling material, utilized for cosmetic dental restorations.  Composite fillings may be used in the situations of a broken tooth or teeth, enamel fractures, cracked teeth, chipped teeth, space closures, and teeth affected by decay, lost or broken fillings and trauma.  The decay of the tooth, week unsupported tooth structure and weaken enamel is removed at the time the tooth is prepared for the filling to be placed by the dentist and dental team. The tooth is treated with antibacterial agents, dental bonding and enamel bonding agents and is finally restored using the adhesive like cosmetic composite filling. The Composite filling is the placed in the tooth, sculpted, shaped, contour and cured utilizing ultraviolet light curing technology to set and harden the filling prior to finishing and polishing the final restoration. The Final filling is check so that the proper biting relationship is established and is comfortable for the patient. The composite materials are utilized in a variety of cosmetic dentistry applications. Excerpt   Dental Enamel, the hardest substance in our body, is 96% mineral, and is non-regenerative and non-reparative.  The dental restorations, bondable ceramic or bonded cosmetic composite fillings serves not only to restore the natural esthetics of the tooth, but to protect the underlying highly innervated less mineralized dentin and soft vascular pulpal tissues. These restorations or dental fillings protect remaining tooth structure, from the oral environment and improve remaining support to the enamel ,thus enhancing tooth strength, to avoid further breakdown of the remaining tooth structure

The Enamel

The enamel portion of the tooth, is an extremely hard, highly mineralized, non-cellular material that gives the tooth its cosmetic properties and the hard, wear-resistant surface we chew our food with. The enamel and its’ cosmetic characteristics, are formed at the time the tooth develops within the jaw.

Enamel, the hardest substance in our body, is 96% mineral, and is non-regenerative and non-reparative.  Unlike bone and the internal dentin of our tooth, enamel does not contain any collagen fibers and significantle contributes to its rather brittle less flexible quality. Enamel provides significant hardness, yet very brittle qualities and is prone to fracture, especially when our tooth is traumatized or structually compromised.

 These characteristics of enamel are significant in that heavy wear and grinding, trauma, and decay, is highly damaging to the tooth strength and  as well as its cosmetics. Although a little enamel loss is tolerable, significant loss from destructive forces will require dental restoration to protect the remaining tooth structure.  Most enamel in the permanent dentition has completed mineralization during the first year of your life, beyond which your body is not able to form new enamel or repair any enamel that may be subject to destructive forces.  It is therefor recommended that prudent steps be take to care of  the dental enamel,

The optical properties of the enamel hydroxylapatite mineral, provide the cosmetic characteristics of the tooth in such a way as to allow light to transmit through the enamel layer of the tooth, befor being reflected off of the internal dentin layer.  This translucent nature of the dental enamel allows the light to transmit through and reflect off the dentin in such a way that it is precieved by the human eye, to have a  translucence and opalescent luster.

The purpose of dentin is to provide the structural foundation to the harder, outer enamel.  Dentin provides a protective layer to the soft internal pulpal tissue of the tooth containing the nerve and blood supply. Dentin is very vascular and cellular, providing the nutrients to tooth from the jaw through its roots.  Dentin, is only 70% mineral, and is much less mineralized than enamel.  

As we age, the dentin continues to form internally in the pulp chamber of the tooth this continuous process of forming dentin, in the vital tooth, provides added protection to the tooth and pulp tissues as the harder enamel is lost from wear caused by abrasion, grinding, clenching, decay, or chemical erosion.  The dentin provides the tooth its’ natural warm bio-esthetic beauty and  internal color, while providing the tooth its’ constant protection against harmful stimuli.  The natural cosmetic variations of the teeth and many or the individual internal characteristics, warm natural tooth colors is related to the dentinal color , its contribution and the variations in the  color and thicknesses of enamel layer above.

Regular Oral Health Care / Preventative Dental Care.

Dental deposits or calculus are a calcified form of plaque and are composed primarily of calcium phosphate, mineral salt deposits and microorganisms creating a mineralized layer attached directly to your teeth.  Plaque covers this mineralized calculus and is heavily populated with a variety of microorganisms.  The calculus formations may be found above and below the gums. 

Dental calculus varies in location, consistency, and is destructive to teeth, gums, and surrounding bone due to the bacteria associated with it. This process known as periodontal disease (periodontitis or gum disease) has far reaching consequences to the patient.  The patient is at risk due to the active disease process and also due to the multiple systemic health risk factors that have more recently been associated with gum disease.

Periodontitis, also referred to as pyorrhea, is a complex disease which can have active acute periods of gingival (gum) infections leading to in-flamed, swollen, bleeding and often painful gums.  During the active acute phase the gums may even show signs of exudate, or puss drainage, around the teeth in more severe cases.  

Periodontitis may also be a chronic disease involving the gums with active periods of flare-ups. During the active phase of the disease, inflammation of the gum tissue results in destruction of adjacent bone that holds the teeth in the jaw.  The inflammatory process of the gum and bone is the most common cause of tooth loss for most adults.

Today it is significant to point out that periodontal disease has associated risks for several other health conditions including heart disease, strokes, diabetes and hormonal imbalances.  Patient’s should be aware that most cardiologists recognize this association and the need for regular and preventative dental health programs.

Recent studies indicate that the status of your oral health provide many initial warning signals to other systemic diseases and more serious health conditions.  Systemic health risks  include heart disease, strokes, uncontrolled diabetes and hormonal imbalances are to name a few.  Often these conditions can be identified early through a comprehensive dental evaluation and  assessing any possible symptoms you may be experiencing.

Patients having already been diagnosed with systemic health conditions, should be aware of the importance in reducing their health risks, through proper dental care, regular dental cleanings, and obtaining require dental treatment on a timely basis. Most importantly all patients should have a better appreciation of the value of their periodic dental examination to screen for possible “red flags to your overall health” or changes that may be occurring in your health status.

Today it is essential that medical and dental professional, recognize the importance of working closely together for all professionals to monitor their patients in a multidisciplinary health approach for the benefit of all, especially their patients.

A large number of systemic diseases, perhaps as great as 90% or more, as suggested by current research, have associated oral symptoms.

It is has been identified by many cardiologist, that the association of

periodontal gum disease and the chronic inflammation(gingivitis) associated with gum disease are “risk factors” for cardiovascular disease.  Regular dental evaluations and cleanings are prudent and perhaps the easiest way to bring about early diagnosis and treatment of both the periodontal condition and possible cardiovascular disease.

Oral bacteria present in the mouth are provided easy access to our bodies through our gums. Patients with chronic gum disease place these patients at a substantially higher risk for heart attack. Gingivitis an earlier form of gum disease and periodontitis a latter form of gum disease having multiple stages; is a direct result of the formations of  active bacteria, build up of bacterial plaque and calculus on your teeth. 

Bacterial colonies from infected gums are able to dislodge from your gums, enter the bloodstream, attach to internal blood vessel walls and lead to further clot formation once established with in the affected blood vessel.  Clot formation on the internal vessel walls result in decreased blood flow to your heart and other organs where the affected vessel are located.  In the heart, these resulting clots increase blood pressure and the patients risk of a heart attack.  

When considering these recent cardiovascular studies We recommend that patients view dental gum disease as a serious systemic health condition, seek early diagnosis and treatment.  Gum disease requires early preventions and intervention, systemic health considerations must always be considered and will require the collaborative assistance of the patients, medical and dental professionals.

If  you have already been diagnosed with heart disease, It is recommended that you  establish regular dental evaluations and maintenance therapy and establish excellent personnel oral hygiene regimen. Intervals on 3 to 4 months is recommended and twice annually at the minimum.

Patients are asked to provide regular medical updates of all medications being taken and medical treatments they are receiving since their last dental evaluation and at each visit.

It is recommended that you follow directions carefully by your physician, cardiologist and Dentists to avoid undesirable consequences. Take all prescribed medications as directed. Ask for assistance when ever you are unsure as to the appropriate course of action.

The Invisalign Orthodontic Alignment System, is often referred to as “clear braces”.  This unique, advanced, orthodontic dental technology is highly effective at realigning natural and restored teeth: improving smiles in a number of different cosmetic alignment and crowding applications.

 The older, more familiar traditional orthodontic method of dental alignment utilizing metal braces and wires, is becoming the less utilized choice, by patients since the introduction of this newer clear alignment system.  Although not all patients may be able to utilize comfortable, advanced orthodontic alignment system, all patients are able to visually assess treatment concerns prior to commencing with orthodontic cosmetic treatment and smile enhancement.

The Invisalign orthodontic alignment system offers many significant advantages for the adult and younger adult patients.  Invisalign provides orthodontic smile enhancement with predictable results, producing beautiful natural smiles in a very efficient and typically much more comfortable treatment approach. 

In addition to the typical crowding and alignment of crowded or crooked teeth, probably one of the greatest, most beneficial applications today, is the utilization of Invisalign for the patients that have already worn braces in the past and have noticed tooth movement or changes since having their braces removed. Tooth movement and cosmetic changes is an unfortunate consequence for the older adult which is easily addressed using the Invisalign method of realignment.  Typically patients that have worn braces for a number of years and are now noticing some minor or even moderate crowding issues (often referred to as relapse by dental professionals) are pleased to learn that these cosmetic issues can be corrected utilizing Invisalign clear orthodontic aligners.

Working adults find this clear Invisagn method of orthodontic alignment and cosmetic correction to be fast, efficient and a very comfortable way of reestablishing that beautiful, natural smile.  A smile they once enjoyed and after wearing braces so many years to accomplish, during the patient’s earlier years of orthodontic treatment.  Invisalign treatment offers an extremely comfortable and uncompromised advantage for the working adult. For working adults, not willing to accept the unsightly placement of metal braces to accomplish the realignment, “Invisalign offers a clear advantage.”

The Invisalign program, is an excellent system for orthodontic corrective cosmetic treatment.  Invisalign treatment requires very few appointments for patients to get started.  Most cases, can be completed in a very short 6 to 9 months of active treatment time, with very few office visits,  This is a big time savings for the busy patient and busy parents of patients.  The Invisalign system of clear aligners are becoming more popular with young adults as well, for understandable and the obvious reasons of not having to wear metal braces.  More and more young people are finding the Invisalign program an appealing alternative, with faster treatment times and more significantly more efficient treatment process.

The Invisalign system offers a popular, state of the art, computer aided simulation program for allowing our patients to view their smile and assess their smile before and after treatment.  Invisalign accomplished this through a unique CAT scan technology of the patients intial dental molds prior to beginning treatment.  The Invisalign computer software allows patients to view their actual teeth, after being uploaded onto the companies proprietary software, allowing the patient and doctor to analyses their dentition right along with the dental profession.  This effectively allows the patients dentition to be analyzed in many different 3 dimensional views.  Smiles can be evaluated in the many views, dental concerns can be visually expressed and addressed, and the final orthodontic aligners to be fabricated in the event Invisalign is chosen as the treatment of choice to enhace the patients smile.
